Guidelines
1. All dogs must be up to date on Rabies and Distemper Vaccinations. Bordetella Highly Recommended. Please email
proof of vaccinations ahead of time to crsundogs@gmail.com. No dogs on the premises without vaccinations.
2. Liability waiver (below) must be signed and emailed to crsundogs@gmail.com prior to participating (or signed on the booking app)
3. In the email please tell us what dogs you are bringing and describe their current dock experience. Dogs /handlers
with no previous dock diving experience must schedule their first session with Connie or Chuck.
4. Dogs must be kept on leash when not on the dock. Property fences are 4 ft high. Make sure to close the gate behind
you upon entering/leaving. No flexi leads please.
5. Potty dogs prior to going up on the dock (do not let them potty on the pool, dock, or structures/items within the
fence). Clean up after your dog at all times.
6. No food/treats on the dock or in the pool: Lickable treats may be approved – check with owners.
7. Do not feed your dog 2-3 hours before your pool time.
8. Please brush dogs with long fur or thick double coats prior to your pool time.
9. No female dogs in heat.
10. Be mindful of your scheduled time as there may be another pool reservation after you. Please arrive 5-10 minutes
early so you can get in and started on time. Late arrivals are not guaranteed the entire pool time as there may be
another appointment after you. Day of cancellations will be charged full price unless weather at our location
prevents safe use of the pool.
Things to Bring;
1. Your dog’s favorite FLOATING toy or bumper. No live items or food may be taken on the dog or thrown in the pool.
2. Towels to dry off. Dryers not available at this time.
3. Life jackets may be brought. There are some available to use: please return them to their hook when finished.
4. Be aware: you may get wet.
5. Kennel for your dog if you will need to leave them, rotate dogs, etc. Kennels not provided.

Liability Waiver for use of Pool and Dock
I acknowledge that there are inherent risks involved with the sport of canine dock diving, as well as swimming
which could result in bodily injury to my dog and/or myself. I accept and assume all risks and all losses, costs
and damages incurred in regards to my dog’s participation on the dock, in the pool, and on the property of
Charles and Connie Adams, 4391 Sundance Drive, Cedar Rapids, IA 52411.
I accept that Charles and Connie Adams are not responsible for lost, stolen, or damages to personal property
belonging to me/my dog. I also agree that I am responsible for the cost of any damage to the property, pool, or
dock, by my dog or myself.
I understand that my pet must be in good health to participate, and free of fleas/ticks.
I agree by my signature that I am aware of the above policies and the guidelines set forth.
I agree by my signature that I have read, understand, and agree to the policies, regulations and liabilities listed
in this agreement. I agree to hold Charles and Connie Adams, CR Sundogs, its members, volunteers, the owner
of the premises, and any party or employee of the aforementioned parties, harmless from any claim or loss
which may be alleged to have been caused directly or indirectly to any person, animal or things by this dog
while in or upon the premises or near the entrance thereto.
I personally assume all responsibility and liability for any such claim. I further agree to hold aforementioned
parties harmless for any such claim for the loss of this dog due to disappearance, theft, damage or injury or any
other causes. I am solely responsible for my children’s safety and welfare as well as their supervision during the
entire time if they are present on the grounds.
